Request: Lyme Disease

I have Chronic Lyme, and I just don't feel like I understand this well enough to make the life factor in the time-frame (aka, uh, any time soon) I rather need it for to track my health. I can provide things to track, if someone is willing to; I'm hoping that once I have this, I'll be able to look at it and possibly modify it, since there are some cross-over elements, to create one for bartonella and one for babesia, since I rather desperately and seriously need all three.

Thanks, to anyone would could help--I don't want to be greedy, so I'm just asking for the Lyme. I'm hoping I can figure out the other two with modifying the first.
